Oral health status among hospitalized patients

Abstract:

Aim: To investigate into oral health status and its association with health status in hospitalized patients.

Methods: A total of 82 patients were examined and 49 (59.7%) patients were men. The patients answered a survey and oral examinations to detect the number of teeth, oral hygiene index, prostheses hygiene, oral lesions, caries, dental plaque index (DPI), gingival inflammation index (GI), gingival bleeding index, periodontitis and periodontal index.

Results: Oral hygiene was associated with age, but it was not related to physical disability. Difficulty eating was mainly associated with age and tooth loss. All full and partially dentate patients presented dental plaque, 38 (69%) poor oral hygiene, 58 (98.1%) gingival  inflammation, 41 (74.5%) periodontal disease and 33 (60%) caries. Oral lesions were detected in 30 (36.5%) and candidiasis (n = 16, 19.6%) was the most frequent mucous lesion. Caries were associated with smoking and poor oral hygiene. Hospital length of stay and age were associated with increased DPI and GI.

Conclusions: The majority of hospitalized patients did not present satisfactory oral hygiene. Caries and periodontal diseases are associated with health behaviours. Increased time length at hospital could increase gingival inflammation and dental plaque accumulation.

RESUMO

 

Neste artigo de artigo de revisão bibliográfica, abordamos os possíveis desdobramentos dos quadros infecciosos de origem odontológica, do ponto de vista imunológico.

Assim, estão descritos os mecanismos iniciais de defesa, a resposta induzida englobando a reação inflamatória, a geração da resposta adaptativa ou específica, o envolvimento sistêmico e, finalmente, os casos graves que, inclusive, podem levar o paciente a óbito.

Abstract

A paucity of epidemiologic research exists regarding systemic health consequences of endodontic disease.

This study evaluated whether incident radiographically evident lesions of endodontic origin were related to development of coronary heart disease (CHD) among 708 male participants in the VA Dental Longitudinal Study.

At baseline and every three years for up to 32 years, participants (who were not VA patients) received complete medical and dental examinations, including fullmouth radiographs.

Cox regression models estimated the relationship between incident lesions of endodontic origin and time to CHD diagnosis. Among those ? 40 years old, incident lesions of endodontic origin were significantly associated with time to CHD diagnosis (p < 0.05), after adjustment for covariates of interest, with hazard ratios decreasing as age increased.

Among those > 40 years old, no statistically significant association was observed. These findings are consistent with research that suggests relationships between chronic periodontal inflammation and the development of CHD, especially among younger men.

The Department of Periodontics and Oral Medicine of the University of Michigan School of Dentistry (www.dent.umich.edu/pom) invites applications for a full-time clinical track (non-tenure) faculty position at the Clinical Assistant or Clinical Associate Professor level.  Applicants must hold a DDS/DMD or equivalent degree and should have advanced training and/or experience in oral and maxillofacial radiology, oral medicine, and/or comprehensive treatment planning. The Department seeks individuals with demonstrated excellence in both clinical and didactic teaching and a commitment to dental education particularly at the predoctoral and dental hygiene levels.  

The Department has an active mentorship program and will provide opportunities for patient care and the development of independent and/or collaborative research programs.
